I've sussed it, he wanted to do dumbell presses but could only find a barbell so decided to do twists at the same time.

the guy surely has injuries or disability. His feet are in different positions, his right leg looks a bit monged (not in the Morecambe sense of the word) so he's obviously doing what he can with his disability...I think.
