that is probably down to hard training rather than high calories, however that is a lot just from shakes!

Not certain, but reactions may be due to insufficient salt intake. If you are training hard you may be sweating out more salt than you are taking in. My daughter did some research on this for her nutrition course at college and if memory serves me the general diet of a strongman was fairly high in salt.

This extract from The Salt Association:

What happens to my body if I consume too little salt? Would I have any symptoms?
Without salt, our bodies cannot function. Sodium depletion is usually associated with dehydration as, when we overheat, we lose water and salt through sweat. Failure to replenish the salt can be harmful. Symptoms of insufficient intake can range from muscular weakness, cramps and heat exhaustion to – in extreme cases – death.

Added something to my post from the internet, maybe any salty food would do, even crisps or a big bag of chips with plenty of salt and vinegar!!!

add sea salt to as many meals as u can. wen ur working and training hard you lose salts thru sweat. try get 5l of water in aswell so not to dehydrate. other thing may be to do with the lactose in the shakes, my stomach cramps if drink more than 2l of milk per day. everyones body is different and cant all follow same diets as others.

5L is absurd and can result in having a low sodium concentration since you're getting diluted. You'll also be sweating off a lot of salt, which is not replaced when drinking an extreme amount of water like that.

You are already getting PLENTY of water from your shakes. Milk is mostly water.

Drink when thirsty, you have a fully functioning thirst regulating mechanism for a reason.

Whoops, didn't read properly. I see now the OP is not drinking 5L of water. Don't do that.

4to 5l is average a hard training hard working person should consume. water helps keep the kidneys clean out for a start. and from all the protein that trainers consume kidneys take some hammer processing it all. fair enuf drink a litre or 2 if u sit in an office allday and dont train, but if u do train waters one a the most important things behind nutrition.

4000kcal from shakes alone is crazy mate, absolutly crazy!

Its very unlikely you need more salt in your diet.

Its very unlikely you need 4/5L of water per day.

Can't really help with the question though.

Experience: 1st in nutrition

Just to add to the above:

What do youre shakes contain? There must be a SHED load of sugars and fats in those shakes.

Where are you getting this from? Sedentary people don't need 2L of water either. 5L is excessive, though probably not enough to cause actual illness. Though if your theory that protein hurts the kidneys is true (it isn't), then drinking excess water is the last thing you'd want to do.

Where's the data that says you need 5L of water? I'm sure it's been studied extensively in elite athletes.

Your Dr's advice for you, in a diseased state, may not be appropriate for other [healthy] people.
